Periodontal Health in Special-Care Homes in Altenburg/Thuringia/Germany
Objectives: To study the periodontal health of residents of three of four special care homes in the former ducal residence town with 31 thousand inhibitants. Methods: After obtaining ethical approval all four special-care homes were contacted. All residents willing to participate answered a questionaire and were clinically examined. Several items/information were extracted from the existing documents of the residence. Partly the acquiescence was given by the authorized representative. The following data were registered: age, duration of the stay in the residence, grade of toughness/resilience=degree of care necessity (grade 1-5), diabetes mellitus, dementia, body-mass-index. During the clinical examination the following variables were recorded: CPITN, number of teeth, oral hygiene index, AL, PD, BOP, tooth mobility as well as stage and grade of periodontitis. The chi2- square-test, Kruskal-Wallis- and Mann-Whitney-U-tests as well as binary logistic regression were used for the statistical analysis. Results: 179 residents (62.8%, 141 females and 38 males, mean age: woman 84.2 years, men 78.4 years) in three of four special-care homes could be examined. The mean duration of the stay in the residence was 3.6 years. The degree of care necessity was as follows: grade 2-23.5%, 3-34.1%, 4-31.3%, 5-11.2%. 49 residents were severe disabled. The prevalence of diabetes was 44.1%, dementia 43.6%, adiposity 20.1%. 93.4% of the residents were non-smokers. The periodontal variables could be recorded in 72 of 99 dentated residents. The number of teeth was 1-31. The prevalence of AL ≥ 5 mm was 85.7%, of PD ≥ 6 mm 31.9%, 59.7% of tooth mobility grade 1, 19.4% grade 2 and 13.9% grade 3. 33.9% of the residents had no tooth mobility. The EFP-periodontitis stages were as follows: I-3.0%, II–20.9%, III–6.0%, IV–70.1%; the grades were: A–6.0%, B–86.6%, C–7.5%. Conclusions: Within the limits of the present study the study suggests that a high percentage of the residents of these special-care homes has a poor periodontal health. Special care and resources as well as well-designed guidelines could improve the periodontal health of these persons.
