CARIES, PERIODONTAL DISEASE and PERIODONTOPATIC FEATURES in EXTREME MATERNAL MORBIDITY

Objectives: To establish dental, periodontal and microbiological characteristics in pregnant with and without extreme maternal morbidity hospitalized
Methods: A case-control study was designed comparing sociodemographic conditions, dental, periodontal and subgingival microbiological characteristics in EMM as cases and in hospitalized patients without extreme maternal morbidity (NEMM) as controls, at one University Hospital. Patients were classified for EMM by institutional gynecologists with the criteria established by the Colombian National Institute of Health.
Results: 34 EMM cases and 35 controls were included and the most prevalent disease-specific criterion for EMM was severe preeclampsia (70.6%); while the most prevalent diagnosis was non-severe preeclampsia (57.1%) and premature rupture of membranes (20%) in controls. Caries prevalence and DMFT score were similar among study groups. Periodontal disease affected all study population and moderate and severe periodontitis was more prevalent in EMM (85.3%) as compared to NEMM (17.14%) In contrast, gingivitis and slight periodontitis was prevalent in NEMM (82.86%) as compared to EMM (14.7%). In the logistic model, periodontitis severity as well as T. forsythensis, P. micra, Campylobacter species and Gram-negative enteric bacilli were associated with EMM.
Conclusions: This study revealed a higher severity of periodontal disease, and presence of some periodontopathic species in EMM but the exact pathogenetic mechanisms linking both conditions is still unknown
