Self-Reported Periodontitis and Incident Diabetes Mellitus: a Nation-Wide Cohort Study
Objectives: To evaluate the association between severe periodontitis (evaluated by means of a previously validated questionnaire) and the diagnosis of incident diabetes mellitus (DM), in a representative sample of the Spanish population. Methods: The di@bet.es epidemiological study is a population-based cohort study aimed to determine the prevalence and incidence of DM in the adult population of Spain. The baseline cross-sectional examination undertaken between 2008 and 2010 using a random cluster sampling of the Spanish population included 5,072 subjects and established the prevalence of DM in Spain. The cohort was re-evaluated 7.5±0.6 years later to determine the incidence of DM. The at-risk sample included 4,347 subjects, 2,408 completed the follow up, and 2020 of them completed the self-reported periodontitis questionnaire proposed by the Centers for Disease Control and prevention (CDC) and the American Academy of Periodontology (AAP), that had been previously validated in the Spanish population. Results: In total, 144 subjects developed DM, which yields a 7.1% cumulative incidence. Self-reported severe periodontitis was detected in 707 subjects (35.0%), 63 of them presenting incident DM (43.8% among those developing DM; p=0.022). Logistic regression analyses showed that self-reported severe periodontitis was associated with incident DM (odds ratio=1.5; 95% confidence interval [1.1; 2.1]; p=0.023). After adjusting for significant confounders, the association was attenuated. Specifically, age stratification showed that the association between incident DM and severe periodontitis was stronger in older subjects. Conclusions: The results of the present study suggest that periodontitis is associated with incident DM in the Spanish population.
