Mechanism Of Adult Anterior Open Bite Treatment Using Clear Aligners

Objectives: To evaluate both dental and skeletal changes after clear aligner treatment of anterior open bite in Class I, II, and III malocclusion adult patients.
Methods: In this retrospective study, three treatment groups of adults (≥18 years old) with anterior open bites treated with clear aligners were included: 24 Class I patients, 16 Class II patients, and 9 Class III patients. Both dental and skeletal cephalometric measurements were reported for each group. A paired t-test was used to assess dental and skeletal changes in closing the anterior open bite. ANOVA was used to compare treatment effects according to Class I, II, and III groups.
Results: Interrater reliability was appropriate with an ICC of 0.9. The main mechanism of open bite closure using clear aligners was retroclination of upper incisors (-5.90±4.72°, p<0.001) and lower incisors (-4.89±5.30°, p<0.001), with subsequent extrusion of the upper incisors (1.04±0.93mm, p<0.001) and lower incisors (1.55±1.40mm, p<0.001). When Class I, II, and III malocclusion groups were compared, upper molar intrusion was significantly greater in Class I and II groups (p<0.001). Lower incisor retroclination and extrusion, on the other hand, were significant in Class III group (p<0.05).
Conclusions: After open bite treatment with clear aligners, upper and lower incisors were retracted and extruded to close the bite. There was a significant difference in the treatment mechanism between Class II and Class III groups. Class II patients presented with significant upper molar intrusion which led to the autorotation of the mandible. Class III group presented with significant retroclination and extrusion of the lower incisors in correcting the anterior open bite.
