TMD Pain Screener: A Marker of Acute to Chronic Painful Temporomandibular Disorders Transition

Objectives: The aim of this cohort study was to assess the impact of the TMD pain screener items in the transition from acute to chronic painful temporomandibular disorders (TMD) within 6 months of follow-up.
Methods: Eight-two participants with acute painful TMD (< 3 months) were recruited from four dental clinics in Montreal, Canada: Oral Diagnosis clinic of McGill University, Orofacial Pain clinic of the Montreal General Hospital, the dental clinic of the JGH and a dental clinic in Ottawa. Participants received the diagnosis of TMD based on the Diagnostic Criteria and completed the TMD screener and the Graded Chronic Pain Scale at 1, 3 and 6 months of follow-ups. Depression was assessed at baseline using the Personal Health Questionnaire Depression Scale. The association between TMD screener items and the transition from acute to chronic TMD pain at 3 and 6 months of follow-up was assessed through a longitudinal analysis using SAS PROC GENMOD repeated analysis.
Results: Pain or stiffness on awakening (RR=1.66, P<0.0001), chewing hard or tough food (RR=2.39, P<0.0001), opening (RR=1.82, P<0.0001), jaw habits (RR = 1.67, P<0.0001) and other activities (RR = 1.58, P<0.0001) were associated with acute to chronic TMD pain transition during follow-up. Pain or stiffness on awakening (RR=1.19, P=0.03), and chewing (RR=2.07, P<0.0001) remained associated with the transition when the multivariable model included all items. Age, gender and depression were not related to the acute to chronic TMD pain transition during follow-up.
Conclusions: The TMD screener items are markers associated with transition risk, regardless of depression status, age and sex covariates.
