Evaluation of the Maxillary Sinus After Maxillary Expansion by CBCT
Objectives: The aim of this study was to evaluate changes in the maxillary sinus after orthodontic treatment with expansion using cone beam computed tomography (CBCT). Methods: Twenty patients who had rapid palatal expansion (RPE) as part of their orthodontic treatment were examined. Each patient had CBCT scans (NewTom 5G, Italy) taken before (T1) and after (T2) RPE. Linear and angular measurements were made to determine the effects of dental and skeletal expansion. The dimensional, volumetric, and color map evaluations of the maxillary sinus were also evaluated using the Simpleware ScanIP(TM) software (California, USA). Forty CBCTs were evaluated and the data (T1 vs T2) were statistically analyzed using the intraclass correlation coefficient (ICC) and the one sample Wilcoxon Signed Rank test at α = 0.05. Results: Strong ICCs were recorded at both T1 (0.964) and T2 (0.970) indicating that the method of measurement was highly reliable reproducible. Using one sample Wilcoxon Signed Rank test significant differences were observed between T1 and T2 for all measurements (p < .05) except antero-posterior length (p = .896), height of palatal vault (p = .067), palatal vault angel (p = .332), left maxillary sinus width (p = .191) and right maxillary sinus width (p = .218). Color map showed lateral displacement of the maxillary sinus after treatment (p < .05). Conclusions: These results suggest, except for the width with lateral displacement, RPE increases all dimensions of the maxillary sinus and may also result in skeletal changes.
