Inferior Alveolar Canal and Relation to Mandibular Posterior Teeth

Objectives: Iatrogenic inferior alveolar nerve (IAN) injuries can occur due to a periapical procedure of the mandibular posterior teeth or during implant placement due to proximity of the IAC. The goal of the project is to identify anatomic relationship of mandibular molar and premolar teeth root apex to the roof of the IAN as well as the relationship of the canal to the buccal and the lingual cortices of the mandible using Cone-beam Computed Tomography (CBCT) volumes.
Methods: Retrospective analysis of the CBCT images obtained between 2013 to 2019 are being analyzed. Distance between the IAN and the root apices of the mandibular molars and premolars are measured from: roof of mandibular canal. IAC relation to buccal cortical and lingual cortical plates is measured on both the right and the left side.
Results:
On an average the distance between the lingual cortical plate to the IAC at the molar region was 0.98 (±0.23) and the buccal cortical plate to IAC was 3.4(±0.32). The root apex of tooth #31 mesial 2.28 (±0.87) distal root 3.35 (±0.96) tooth #30 mesial root 3.2(±0.77) and distal root 3.33(±0.57).
Conclusions: The current data suggests close proximity of lingual cortices to IAC near the second and first molars and bucally nears the premolars due to the mental foramen. Mesial root apex of tooth #31 mesial is in close proximity to IAC compared to the distal root apex and similarly tooth #30 mesial root apex appears to be more in close proximity to IAC roof compared to the distal root apex. Dentoalveolar surgery, implant planning must be carefully considered for potential nerve injury.
