Dental Implant Surfaces Change After Insertion in Dense Bone.
Objectives: Stress concentration on bone during implant placement may compromise the integrity of the surface features and consequently release titanium particles in the bone tissue. To evaluate the surface damage to different dental implants caused by the insertion procedure in dense bone in vitro. Methods: Thirty implants with different thread designs (Anker/n=5; Ankylos/n=5; Intralock/n=5; Nobel Replace/n=5; Straumann BLT/n=5; Straumann TE/n=5), were placed in artificial bone (SimBone) representing type I dense bone quality following the insertion protocol recommended by each manufacturer. Different roughness parameters (Sa, Sz, Sq, Sdr, and Spd) were evaluated before and after the implant insertion at the thread peaks in four different locations: neck, coronal third, middle third and apical third, using a Laser Confocal Microscope (Keyence-VK-X150) with a magnification of 20X. Statistical analysis was performed using the t-test for two related samples (before and after insertion). Results: The implant insertion torque values were 50-60 Ncm. All the roughness parameters showed statistically significant differences in each of the six implant systems (t-test, p〈0.05). The implant surface roughness was diminished after insertion in type I bone. Results: The implant insertion torque values were 50-60 Ncm. Conclusions: The implant surface roughness changes after implant insertion in dense bone when the protocol recommended by the manufacturer is followed. The observed surface changes indicate surface deformation and potentially titanium particles release during the implant insertion.
