The Efficacy of a New Power Toothbrush in Reducing Gingivitis

Objectives: This 12-week study evaluated a new power-toothbrush compared to a manual brush (Control) for reductions in gingivitis in a non-flossing population repeatedly trained in a brushing technique appropriate to each toothbrush.

Methods: Eligible subjects were randomized, and at baseline, two and four weeks were trained to use their assigned toothbrush. The round-head oscillating-rotating brush with tapered filaments was compared to an ADA referenced flat trim manual toothbrush. The Lobene Modified Gingival Index (LMGI), probing depth (PPD), bleeding on probing (BOP), and Turesky Plaque index (TPI) were assessed in order after overnight plaque formation at the baseline, 2, 4 and 12-week visits, with TPI carried out Pre- and Post-brushing at baseline. Each participant received a two-minute timer to ensure equivalent brushing time. Paired t-test and repeated measure ANOVA mixed model were used for intra- and inter-group comparisons over time. Frequency analysis was done using chi-square test.

Results: Of 58 participants, 51 completed the trial. Baseline mean LMGI and TPI values were equal, while mean BOP was higher in controls at baseline, without statistical significance. Both groups showed improvements for all indices at all visits. While BOP showed more improvement in the test group compared to control group the baseline imbalance may contribute. When only sites with bleeding at baseline were compared, the test group showed higher reduction in BOP compared to the control (p<0.01).

Conclusions: Both groups showed improvement in BOP, LMGI and TPI over 12-weeks. BOP in the test group showed overall better improvement. When the analysis was limited to bleeding sites at baseline, the test brush showed a significant reduction in BOP compared to control. Gingivitis can be reduced in the short term by improved brushing without additional oral hygiene aids. The power toothbrush is slightly more effective at reducing BOP than a manual toothbrush.
