Randomized Controlled Clinical Comparison Of Vital Pulp Capping Protocols
Objectives: To compare the pulp capping clinical outcomes under indirect and direct restorative protocols on tooth sensitivity and pulp vitality over a period of 12 months. Methods: A randomized, controlled, double-blind, prospective study assessed a total of 109 deep carious lesions in permanent teeth of subjects between 19-64 years old. Three indirect pulp capping protocols (n=30) compared the use of intermediate liners [resin calcium silicate cement (Theracal LC, Bisco) and calcium hydroxide (Dycal, Dentsply)] and no liner (All Bond Universal, selective etching technique). Teeth requiring direct pulp capping (n=10) were restored with either Theracal or Dycal protocols. All preparations were restored with resin composite. The outcomes measures were pain using VAS scale and success rate (pulp vitality based on percussion, palpation, cold test, radiography), collected at screening visit, intervention day, 24 hrs., 7 days, 3 months, 6 months and 12-month follow-up visits. Descriptive data and rates of success were calculated. Statistical analyses of proportions were done using Fisher’s test and 95% confidence intervals. Results: A total of 2 cases (1 Direct and 1 Indirect pulp capping, both using Theracal) failed within 4 months due to irreversible pulpitis. There were no statistically significant difference in self-reported tooth sensitivity among protocols for the indirect (p > 0.1) nor between the direct (p > 0.1) therapies. The respective success rate (pulp remains vital) after 1 year for direct were TheraCal (96.2%), Dycal (100%), and no liner (100%); while direct were TheraCal (86%) and Dycal (100%). There were no statistically significant difference in the success rates among protocols for the indirect (p > 0.1) nor between the direct (p > 0.1) therapies. Conclusions: Based on 1-year evaluation, the success rate of direct and indirect pulp capping therapy in permanent teeth was not affected by the restorative protocol.
