Deinstitutionalization of Adults with Mental Disorders: Impact of Oral Health

Objectives: To describe the oral health epidemiological profile, educational level, self-esteem and their impact on the deinstitutionalization of adults with mental disorders.
Methods: A descriptive cross-sectional study was undertaken in mental health community centers (MHCC) (N=230). The nonprobability sample consisted of adults of both sexes (n=188). An intervention study based on prosthetic rehabilitation was carried out. The independent variables surveyed were (a) sociodemographic data (age, sex, psychiatric disorder, educational level) and (b) management process (institutional stay, readmissions). A numerical value was assigned to the educational level. The observable endogenous variables (oral health and self-esteem) were estimated using the DMFT index and the Rosenberg Self-Esteem Scale (RSES). Surveyed data were transcribed using a Likert-type scale; surveys were repeated 6 months post-intervention. Measures of central tendency and dispersion were calculated for each variable. Student’s t-test was used for DMFT; Spearman’s correlation analysis and Mann–Whitney test for RSES.
Results: Age (mean SD) is 47.14 (9.86). Institutional stay prior to admission to program, 16.40 years (10.96); between sexes (P=0.31); age, institutional stay (r=0.46). Length of stay at MHCC was 6.66 years (4.65); Mann–Whitney test, P=0.13 according to sex. Number of readmissions was 1.67 (2.42), with a significant positive relationship between length of institutional stay and number of readmissions (r=0.27). Educational level shows a value of 2.65 (1.48). No significant relationships were found between educational level and sex (r=−0.02); length of stay and educational level (r=−0.18); age and educational level (r=−0.19). DMFT scores were 19.71 (5.54), significantly higher in females than in males (P=0.03). RSES scores pre- and post-intervention were median 21, IQR 1.5 and 25, IQR 1 (P=0.01); P=0.71 according to sex. Differences between diagnoses, P=1.
Conclusions: A great need for dental treatment and a low educational level were observed. The intervention impacted on the self-esteem, with a low program discharge score.
