Effect of a Toothpaste Containing Oleanolic Acid in Reducing Gingivitis

Objectives: Given the long-term impact of gingivitis on the risk for periodontitis and tooth loss and its high worldwide prevalence, efforts should be made to treat and prevent gingivitis. Based on this, the aim of this study was to assess the efficacy of a oleanolic acid (OA) containing toothpaste in reducing gingival inflammation in patients with gingivitis.
Methods: This double blind, controlled, randomized clinical trial included 82 subjects with moderate gingivitis. Following a 1-week washout, subjects were randomized into three groups: i) OA group (fluoride toothpaste containing 0.1% OA and placebo mouthwash); ii) negative control (fluoride toothpaste and placebo mouthwash); and iii) CHX group (fluoride toothpaste and 0.12% chlorhexidine (CHX) mouthwash). Subjects were clinically assessed at inclusion, baseline, and after 4 days, 1 week and 2 weeks of twice daily use of the allocated products. Subjects received a diary for documentation of bleeding events during brushing. Data was analyzed by t-test, ANOVA and Kruskal- Wallis tests.
Results: After two weeks of product use, all groups showed reductions in gingival index (GI), interproximal GI, gingival severity index (GSI), plaque index (PI) and interproximal PI. The adjunct use of CHX mouthwash promoted additional benefits in plaque and gingivitis control, as compared to sole use of a fluoride toothpaste (negative control). Although a OA containing toothpaste was unable to reduce plaque to the same extent as CHX, it reduced gingival inflammation just as CHX. Negative control and CHX groups failed to reduce self-reported bleeding events on brushing. In sharp contrast, the OA group reported a reduction in self-reported bleeding events.
Conclusions: All groups showed reductions in plaque and gingivitis, albeit not to the same extent. OA toothpaste showed promising trends in controlling gingivitis.
