Twelve-year Clinical Evaluation of Two Types of Resin Composite

Objectives: This randomized controlled clinical trial evaluated 12-year clinical performance of two types of resin composite restored with a one-step self-etch system.
Methods: Ninety-eight non-carious cervical lesions (NCCLs) in 22 subjects with a mean age of 61.9 years (range 29-78) were involved in the study. Enamel bevel was placed and dentin walls were lightly ground, and restored with S3 Bond (Kuraray Noritake Dental, Tokyo, Japan) in conjunction with a hybrid (AP-X: AP, Kuraray) or a flowable (Flow FX: FX, Kuraray) resin composite. Each patient received both restorative groups randomly. All restorations (48 for AP and 50 for FX) were placed by one dentist. The restorations were blindly evaluated every year using modified USPHS criteria by two examiners. The data were statistically analyzed using the Fisher’s exact test and the Kaplan-Meier method.
Results: Nineteen subjects were examined at the 12-year recall. Retention rates for AP and FX were 98% and 90%. Secondary caries was detected around 3 restorations restored with FX. FX showed a significantly poor wear resistance and resulted in deterioration of marginal adaptation. As a result, there was a significant difference in 12-year survival rates between AP and FX. Other clinical problem was marginal staining that increased with time and reached a plateau in around 9 years, showing a 60% incidence regardless of the types of resin composite. There were no significant differences in the clinical performances between AP and FX for other variables, such as sensitivity and color change. It should be noted that the recurrence of NCCL and gum recession, which were not directly related to the restorations, also increased over time.
Conclusions: The findings of this study suggested that the earlier flowable composite demonstrated poorer clinical performance in NCCLs compared to the hybrid composite.
