Creeping Attachment Following Free Gingival Grafts Around Dental Implants
Objectives: The purpose of this study was to assess changes in the level of mucosal margin (MM) following free gingival grafts (FGGs) around implants exhibiting mucosal recession (MR) along with <2 mm keratinized mucosa (KM) compared to implants with ≧2 mm KM for 12 months. Methods: This study was conducted under the approved IRB protocol (HP-00088459). 21 implants displaying implant thread and/or prosthetic abutment exposures with <2 mm KM from 13 subjects were treated with FGGs (the FGG group). The control group comprised 11 implants exhibiting ≧2 mm KM, which received no FGGs, from nine subjects out of the 13 subjects. The width of KM was measured at baseline, 24 and 52 weeks. The amount of MR was measured at baseline, 2, 24, and 52 weeks in the FGG group and at baseline, 24, and 52 weeks in the control group. The two-way ANOVA was conducted to compare the width of KM and the amount of MR between the two groups. Results: The width of KM in the FGG group (0.6±0.6 mm) was significantly less than the control group (2.4±0.5 mm) at baseline. The width of KM in the FGG group (3.4±1.3 mm) was significantly greater than the control group (2.4±0.6 mm) at 52 weeks (F=18.81, p<0.0001). The continuous reduction in the MR was observed at 2, 24, and 52 weeks from baseline in the FGG group (F=4.53, p=0.0055), while no change in the level of MM was observed in the control group from baseline to 52 weeks. Conclusions: In conjunction with gains in the width of KM, postoperative coronal migration of the MM occurred following FGGs around the implants displaying MR with <2 mm KM. This postoperative coronal migration resulted in 0.7 mm mean reduction in the MR at 52 weeks from baseline in the FGG group.
