Evaluation of Lentulospiral  vs Hybrosonic for obturation of primary teeth

Objective: This in vitro study evaluated the quality of obturation  in primary root canals using two different obturation techniques: Rotations using Lentulo spiral and Vibrations using Heat plus sonic vibratory root canal obturating device [(HYBRONIC) filed for patency under the Government of India, INTELLECTUAL PROPERTY OFFICE (IPO)].

Method: 45  root canals from 24 extracted primary teeth (13 incisors and 11 molars) were prepared  up to size 35 with hedstrom files using pull-back action. Wide canals such as those of maxillary incisors were only cleaned and irrigated. The root canal filling material (zinc oxide eugenol paste) was deposited into the canals either by a lentulo spiral mounted in the slow-speed handpiece or by the Heat plus sonic vibratory root canal obturating device. The quality of obturation was evaluated on radiographs and cone beam computer tomography (CBCT) using specific criteria like underfilling, optimal filling, overfilling, presence/number of voids.     

Result: Vibratory technique gave more optimal fillings (72%) compared to lentulo spiral (28%) among the total optimally filled canals whereas it gave less overfillings (35%) compared to lentulo spiral (65%) among the total overfilled canals, the differences being statistically significant (P = 0.019). There was no significant difference between the number of voids produced by the two techniques as seen on radiographs and confirmed on CBCT. 

Conclusion: Overfilling of canals, which may have a significant adverse effect on the clinical success rate, was seen less frequently by the vibratory technique compared to lentulo spiral. Both the techniques showed presence of voids in obturation but this finding may not be clinically significant. The new vibratory technique of obturation [HYBROSONIC] definitely has scope for further research such as comparisons with various other techniques and materials as well as its long term clinical success rate.
