Objectives:
Occlusal caries-preventive effects of fissure sealants are worldly recognized. However, pretreatment with phosphoric acid etching demineralizes sound enamel underneath the sealant-applied region. This raises concerns about secondary caries in demineralized enamel. This study aimed to observe surface morphology of human enamel treated with a novel pit-and-fissure sealant system containing S-PRG filler, and its bonding ability in comparison with conventional sealant system.
Methods:
Materials used are a novel sealant system (SI:SI-R20901) consisting of self-etching primer containing phosphonic adhesive monomer and sealant containing S-PRG filler having fluoride-releasing ability and acid-buffering capacity; and Teeth Mate (TM), involving phosphoric acid etching as control. A bovine enamel surface polished with 600-grit SiC paper was pre-treated according to manufacturer's recommendations. A mold (diameter: 4mm, height: 0.2mm) was placed on the treated surface, filled with sealant paste, and light-cured. After stored in water (37ºC, 24h) and thermal cycling, the specimen was subjected to shear bond strength (SBS) measurement using a universal testing machine (Instron 5567).Also, an intact human enamel surface was treated according to manufacturer's recommendations, rinsed with EtOH, and observed with a laser microscope.

Results:
SI exhibited equal or higher initial SBS and bonding durability to enamel, compared to those of TM. The enamel surface treated with SI's self-etching primer showed less demineralization and less irregular morphology than the surface treated with TM's phosphoric acid etchant. There was no difference in enamel surface morphology before and after treatment with SI's self-etching primer.
Conclusion:
Unlike phosphoric acid etching, SI's self-etching primer did not cause excess demineralization on enamel, and exhibited bonding ability equal to or higher than that of the phosphoric acid etchant. Furthermore, due to the reported biological effectiveness (reinforcement of tooth structure, anti-plaque effect, remineralization of dentin, acid-buffering capacity, etc.) of S-PRG filler, SI is expected to be a new occlusal caries-preventive method.
