IL8 in GCF in Patients with Definitive Full Coverage Restorations

Many studies have reported that the gingival crevicular fluid (GCF) of patients with periodontal disease contains increased levels of cytokines including multiple interleukins. Objectives: The purpose of this study was to determine interleukin (IL)-8 levels in GCF in patients with definitive full coverage restorations under different periodontal conditions. Methods: In this pilot study (UDM IRB #0708-36), multiple GCF samples (a total of 30) were taken from 5 study participants with definitive full coverage restorations with both subgingival and supragingival margins. The periodontal health of the volunteers ranged from slight gingivitis to generalized severe periodontitis. GCF was collected using Periopaper strips (OraFlow, Inc.) and GCF volume was measured using the Periotron 8000. Probing depths and bleeding on probing (BOP) were assessed at clinical examination. IL8 concentration was determined by ELISA, and standardized across all samples against total fluid volume. Results: Within each patient, IL-8 levels were variable. The average IL-8 concentration in GCF in restored teeth in a patient with generalized slight gingivitis, probing depths of 3, and no BOP, was 66.1 ± 15.7 pg/ml. IL-8 concentrations in GCF in restored teeth in patients with slight or moderate periodontitis ranged from 143.6 ± 114.2 pg/ml to 176.9 ± 162.1 pg/ml. The average IL-8 concentration in GCF taken from a patient with definitive full coverage restorations with generalized severe periodontitis, probing depths ranging from 5 to 12, and BOP at several sites, was 215.6 ± 109.0 pg/ml. No significant differences in GCF IL-8 concentration was observed in a comparison of subgingival and supragingival margins of restorations. Conclusions: Gingival inflammation, as assessed by GCF IL-8 concentration, may be more dependent on periodontal health as opposed to presence of definitive full coverage restorations or margin discrepancies.
