Early complications after crown therapy in adolescents with Amelogenesis imperfecta
Longevity of restorations in children with Amelogenesis imperfecta (AI) is limited. Pulpal complications are considered as a risk factor for early prosthetic therapy. Objectives: Is to identify early complications in adolescents and young adults with AI after treatment with ceramic crown restorations. Methods: Prosthetic crown therapy was performed in 20 patients, 8 boys and 12 girls, with AI aged 12-22 years (mean age 17 years). AI type was recorded as either hypoplastic type (10) or hypomineralized or hypomaturated type (10). Dental caries, previous therapy, sensitivity, trauma, endodontic and esthetic status were recorded. After random selection, 129 porcelain crowns with either Procera (62) or IPS E-max (67), both with Zirconia inner copings were cemented with RelyX ARC cement. Sensitivity and pain vas recorded by VAS scale prior to the therapy and one month after therapy. Follow up examination was made one month after cementation. Results: One month after prosthetic therapy no case of pulp involement or increased sensitivity to percussion was found. No loss of crowns was recorded. None of the 129 crowns had esthetical defects. The sensitivity and pain VAS score decreased from 3.6±3.2 before to 1.4±1.8 one month after crown therapy (p=0.0214). Postoperative sensitivity was significantly correlated to the number of ceramic crowns cemented (p=0.0307). Conclusion: This study showed few early complications after early ceramic crowns therapy in adolescents and young adults with AI and reduced sensitivity to pain.
