The German Shortened Dental Arch Study: Three-Year Results
Objectives: The study aimed at gaining reliable data on outcomes and success rates of different treatments in shortened dental arch (SDA) cases. This paper deals with the three year results.
Methods: This ongoing multi-center study (14 dental clinics) was designed as a randomized controlled clinical trial. It is funded by the German Research Association (grant WA 831/2-1-6). The inclusion criteria encompassed a Kennedy Class I topography in one jaw (all molars missing, at least the canine and one premolar present on each side) and the exclusion of implant treatment. Patients were provided with a removable dental prosthesis with precision attachments for molar replacement (RDP group) or retained a premolar occlusion, where required after insertion of fixed restorations (SDA group). In the main study, 215 patients were included. First tooth loss after treatment regardless whether in the study or opposing jaw was the main endpoint. Recalls were scheduled at four to eight weeks (baseline), six months and then annually.
Results: The first tooth loss occurred in an opposing jaw, four months after insertion in the RDP group. At three years, 16 teeth had been extracted in the RDP group compared to 12 teeth in the SDA group. Four tooth losses caused a break down of the treatment concept, two in each group. The Kaplan-Meyer survivor rates related to the main endpoint first tooth loss were 0.75 (95% CI = 0.595 - 0.897) in the RDP group and 0.86 (95% CI = 0.768 - 0.944) in the SDA group. The difference between the survival distributions was not significant (logrank test: p = 0.58).
Conclusion: The frequency of tooth loss in these well monitored study patients was relatively high. With respect to a final conclusion regarding differences between the two treatments, the five year results have to be waited for.
