Postoperative Nausea and Vomiting of Ambulatory Anesthesia for the disabled

Objectives: Postoperative nausea and vomiting (PONV) is a limiting factor in the early discharge of ambulatory anesthesia patients. We retrospectively investigated the incidence of postoperative vomiting and retching (POVR) of ambulatory general anesthesia (GA) for dental treatment of special needs patients and analyzed the cases in order to predict high risk patients and prevent POVR.

Method: 1,100 cases undergone dental treatment under ambulatory GA in an oral health center for the disabled were involved in the investigation. The incidence of POVR, patients' demographic data, methods of anesthesia, duration of treatment and GA, and time until discharge were analyzed.

Results: 670 males and 430 females were involved in the survey. The incidence of POVR was 7.8% (86 cases). Male gender was 62 cases (9.3% of total male) and female gender was 24 cases (5.6% of total female). Average age was 14 years. Anesthesia was induced with sevoflurane in 72 cases and with propofol in 10 cases. Anesthesia was maintained with volatile anesthetics in 85 cases (98.8%) and with propofpl together with nitrous oxide in 1 case. Duration of dental procedure was 115±39min. Duration of GA was 167±42min. Ketamine was administered i.m. as premedication in 2 cases. Intraoperative opioids were not used in any case. Time until discharge was 187±80min.

Conclusions: Society for Ambulatory Anesthesia Guideline indicates risk factors for PONV in adult and children. It was suggested that incidence of POVR in ambulatory GA for special needs dental patient did not follow the patient-specific factors in the Guideline, and largely depended on the induction method of GA and general anesthetics. Influence of nitrous oxide on POVR should be further studied.
