Increased Toluidine Blue Uptake in Oral Leukoplakia from Previous Cancer Sites Predicts Increased Risk of Tumor Development

Objective: The determination of when and where to biopsy oral premalignant lesions (OPLs) at the fragile prior cancer site is a continuing challenge facing clinicians since treatment of cancer induces reactive changes that often resemble OPLs (leukoplakia/erythroplakia). We have recently shown that toluidine blue (TB) staining helps to identify OPLs with high-risk molecular clones in patients without a history of oral cancer. The aim of this study was to investigate whether TB could identify high-risk OPLs in patients with a history of oral cancer.

Method: In an ongoing prospective study, the following information were collected from 59 patients with a history of oral cancer: demographics (age, gender and habit); features of primary oral cancer (TNM staging, histological grading and treatment), and features of OPLs (presence/absence of OPLs; clinical appearance, size, TB staining, pathology and outcome).

Results: Of the 59 patients, 27 developed cancer at the previous cancer sites (called recurrent group) while 32 did not (called non-recurrent group). There were no differences between the recurrent and non-recurrent groups with respect to the demographic characteristics and features of primary oral cancer. OPLs that went on to become recurrent cancer tended to be larger (55% ³ 1 cm versus 27% in nonrecurrent group) and nonhomogeneous (62% versus 27% in nonrecurrent group) although the differences were not significant. There were no differences in the pathology and treatment of OPLs between the two groups. However, 62% of OPLs in the recurrent group stained TB positive as compared to 33% of OPLs in the non-recurrent group (P = 0.05).

Conclusion: TB staining could facilitate the identification of high-risk OPLs in patients with previous history of oral cancer
