Clinical Markers of Oral Manifestations in HIV / AIDS Patients ; Case report

Objectives: Oral manifestation can be clinical marker of systemic disease such as oral candidiasis (OC) and oral hairy leukoplakia (OHL) in patient with viral infection of HIV/AIDS. Oral candidiasis is one of fungal infection caused by Candida albicans which is an oral microflora that can cause opportunistic infection. Oral hairy leukoplakia is epithelial infection in oral caused by Eipstein-Barr Virus (EBV). HIV can increase OHL. This case report describe about oral manifestation of 30-year-old male patient came to RSPAU Dr. Hardjolukito with the chief complaint; limp, cough and fever since a week ago. Weight loss and night sweats continuously since 2 months ago. Intra-oral examination showed a white, multiple pseudomembrane on the buccal mucosa, tongue, palate, uvula and desquamation along the lips. White plaques appeared on the left and right side of the tongue and extended to other parts of the oral cavity since 2 months ago, bitter taste, dry lips, and canker sores (mouth ulcer). Patients underwent a complete blood examination, HIV antibody test, KOH swab examination and direct culture.
Methods: Treatment and observation patient until healing of the lesions.

Conclusions: Oral candidiasis and oral hairy leukoplakia are the first clinical manifestations found in patients with HIV / AIDS, the virus will remain in the body for life even though the virus is not active, but if the immune system decreases due to low CD4+ lymphocytes, the virus can become re-active. Topical nystatin and fluconazole are the preferred antifungal drugs. The best therapy for HIV / AIDS is antiretroviral drugs.
