Oropharyngeal Candidiasis as the Progress of HIV/ AIDS Disease
Objectives: Introduction Oropharyngeal candidiasis (OPC) is an opportunistic fungal infection that is often found in HIV / AIDS patients. Clinical features that are often encountered in OPC HIV patients are trush, erythematous candidiasis, chronic hyperplastic candidiasis and angular cheilitis (perleche). Candida Albicans is the main caused but other candida can also be found. Case A 27-year-old woman came to Airlangga Hospital complaining about a mouth that felt thick and white patch appeared since 2 weeks ago. Lesions have been given mouthwash but have not healed. The patient claimed to lose about 5 kg in weight over the past 3 months and claimed to have been undergoing glandular TB therapy since 6 months ago. On physical examination the patient looks like pale and exhausted. Intraoral conditions appear as white pseudomembranes found on the surface of the tongue, palate, oropharyng, right and left buccal mucosa, diffuse borders, irregular edges, can be scraped and leave erythema. KOH examination, complete blood and anti-HIV have been carried out. Topical antifungal (Nystatin) and fluconazole tablets were prescribed for therapy after the KOH test results showed positive candida albicans in the form of yeast cell and pseudohypae. On anti-HIV examination, reactive results and CD4 examinations were obtained with a total of 92 cells/µL. Methods: Based on the results of clinical examinations, OPC features are often associated with low CD4 counts and are a marker of progression of HIV / AIDS disease. OPC is a problem for people with HIV / AIDS. Fluconazole is the standard therapy for this complaint. In addition to antifungal therapy Retroviral therapy from HIV / AIDS also plays a role in eliminating this complaint. Results: Clinical Case, Not Applicable Conclusions: Clinical Case, Not Applicable
