Efficacy of Botox in Treating Myofascial Pain in Bruxism: a Randomized Trial
Objectives: The aim of this randomized trial was to evaluate changes in the cervico-occlusal angle and pain reduction in patients with bruxism treated with occlusal splints or botulinum toxin. Methods: Thirty-twos patients with pain caused by bruxism were selected and randomly divided into two treatment groups: occlusal splint or botulinum toxin type A injected on both masseter muscles with 60U per person. The patients answered the DC/ TMD questionnaire for diagnosis, assessment of the pain and frequency of clenching or grinding during sleep at baseline and after 3 months. Also, radiographs of the cervical spine were obtained before treatment and after 3 months, where the angle between the vertebrae c3 and c6 was evaluated. Data were tabulated and analyzed with ANOVA MRI and the Bonferroni test for comparisons between groups with p<0.05). Results: There was a statistically significant difference in the intragroup comparison considering reported pain (p=0.001) in all domains of graded chronic pain scale (p < 0.001) and frequency of tooth grinding during sleep (p < 0.001). No differences were observed between the groups. For variation on the cervical angle there was no statistically significant difference (p = 0.193). Conclusions: Both treatments yelded positive results as there was pain and tooth grinding reduction. No effect on the cervical angle was noted.
