Extraction Socket Graft using rhBMP-2, PTFE Membrane, Platelet Rich Fibrin (PRF) and Blood Clot: A Volumetric Controlled Study
Objectives: Tridimensional changes of alveolar sockets after tooth extraction may result in esthetic and functional defects that jeopardize dental implant placement. Several biomaterials have been used to avoid or to reduce these alterations. The aim of this study was to evaluate the effect of recombinant human morphogenetic protein type 2 (rhBMP2), platelet-rich fibrin (PRF), non-resorbable high-density PTFE and a blood clot (control) on extraction socket augmentation.
Methods: A total of 144 subjects with mean age of 44.5 ± 9.3 years with tooth extraction indication of maximum 2 continuous teeth were enrolled in the study. The extraction sockets were divided into 4 groups: control group (blood clot; n = 70 subjects), test groups with graft (rhBMP2; n = 14 subjects / PRF; n= 30 subjects / PTFE; n= 30 subjects). CTBC was taken at 2 days (T0) and four months (T1) after extraction. These images were converted to DICOM and evaluated using a specific software-based on the height of the buccal wall (mm); the palatine wall determined in mm and the area as determined in mm2 obtained through the alveolar process profile (edge), is designed using the cursor and by a software tool. In addition, it was evaluated the intragroup variation in the time intervals (T1 - T0) for the three variables, expressing the reduction variation of buccal height; palatal bone crest and area. Volumetric analysis (mm3) of the alveolar socket contraction was also evaluated after stl. overlapped images at T0 and T1. Results: Comparing T1-T0 for alveolar area, rhBMP-2 and PTFE showed lower reduction, with a significant difference from the control and PRF groups (p <0.001); since blood clot group and PRF showed a loss of area around 30% between T0 and T1, mainly at buccal area. rhBMP-2 and PTFE groups showed lower reduction ranged between 6 to 10% (p<0.05). Conclusions: Within the limits of this study could conclude that recombinant human bone morphogenetic protein-2 (rhBMP-2) and PTFE for alveolar socket preservation resulted in a limited tridimensional contraction of the alveolar ridge after 120 days.
