Evaluation of Condylar and Incisor Position Following Centric Splint Therapy

Objectives: To evaluate the change in condylar and incisor position following treatment with centric relation splint therapy.
Methods: 21 consecutive Class II (12) and Openbite (9) patients planned for orthognathic surgery were enrolled. Subjects wore centric relation splints for 2 months. Pre and post-splint treatment Cone Beam Computed Tomography (CBCT) were taken, reoriented to Frankfort horizontal. Longitudinal CBCTs were registered on the anterior cranial fossa using an automatic voxel-wise registration method. 3D surface models of the condyles and mandibular incisors were built and correspondence surfaces and landmarks were measured. Paired T-Test and ANOVA analysis were performed. After 1 week, 10 cases were randomly selected and remeasured. IntraClass Correlation Coefficient (ICC) analysis was performed.
Results: The average 3D condylar position change was 1.21mm +/- 0.62. The mean position changes in the Transverse, Anteroposterior and Superior-Inferior were 0.42mm, 0.76mm, and 0.60mm respectively. The mean incisor position change was 1.15mm +/- 1.05mm. However, 42% of the subjects showed counterclockwise rotation of the mandible with at least 1.5mm of vertical change in the mandibular incisors and 19% showed vertical incisor changes greater than 3.0mm. There was no statistical significance found for the change in condylar and incisor positions between the openbite and Class II groups. ICC values were 0.92-0.99.
Conclusions: The mean change in condylar position was relatively small, however, one-fifth of the subjects showed significant vertical changes in the mandibular incisor position following centric splint therapy
