Effect of Oral Post-operative Steroids on Horizontal Ridge Augmentation Outcomes

Objectives: The objective of this study was to investigate if the prescription of oral steroids post-operatively has an impact on horizontal ridge augmentation outcomes including resulting implant placement, average gain in horizontal bone width and density and implant placement in the grafted alveolar ridge.
Methods: A retrospective chart review and CBCT analysis was completed to assess the clinical and radiographic outcomes of 53 horizontal ridge augmentation. Information was gathered regarding surgical technique, grafting materials, post-operative healing, bone growth and bone density and dimensional changes as quantified on a CBCT scan. ANCOVA was used to test for differences in bone width and density based on use of post-operative oral steroids, while adjusting for baseline measures.
Results: Of the 53 horizontal ridge augmentation patients, 37% of them were prescribed post-operative oral steroids. All bone grafts were successful and all but one patient were able to have implants placed. The one failure was a patient who did not receive post-operative steroids, but this was not statistically significant (94% vs 100% success, p-value=0.3396). There was an average bone gain of 3.6mm of horizontal augmentation based on radiographic analysis, which did not differ based on the use of steroids (p-value=0.6338). The use of post-operative oral steroids was also not statistically significantly related to increased post-operative bone density (p-value=0.3566).
Conclusions: Incorporating oral steroids into the post-operative care following horizontal ridge augmentation did not make a statistically significant difference in the increase in bone dimensions, density or implant placement. Although post-operative oral steroid therapy did not affect clinical and radiographic outcomes in our study, the steroids may be beneficial in reduction of the inflammatory response. Potential side effects and the negative effect of steroids on healing should also be carefully considered.
