Are Edentulous Patients with Diabetes Receiving Oral Health Evaluations?

Objectives: People with diabetes have higher risk of oral health problems including xerostomia, periodontal disease, and tooth loss. In this research study, we implemented a previously tested diabetes dental quality measure with specifications for electronic health records (EHR) and made modifications to evaluate the type of oral health care delivered to edentulous patients with diabetes.
Methods: Using data available through the BigMouth Dental Data repository, an automated query identified edentulous patients with diabetes who received the following dental procedures: comprehensive or periodic oral evaluation, prosthodontic treatment procedures with diagnostic terminology related to edentulism, oral surgery, or oral pathology procedures in the reporting year. We then performed manual review of patient charts.
Results: In the reporting year of 2017, the query identified 1640 patients with diabetes, and 119 (7.3%) were edentulous. Our validation analysis indicated a sensitivity of 99 percent, specificity and positive predictive value of 100 percent, and negative predictive value of 92 percent. The majority of our patients received prosthodontic treatment procedures (61.3%), followed by oral surgery procedures (32.8%), comprehensive oral evaluation (31.9%), and periodic oral evaluation (21%).
Conclusions: The results from this research study suggest that edentulous patients with diabetes receive oral health care primarily related to prosthodontic procedures. Our results attest the validity of including edentulous patients in the process-of-care quality measures designed to evaluate the oral health care provided to patients with diabetes.
