Manually Made Guided Tooth Preparation: Innovative, Economical And Simple Method

Objectives: Manually developing a simple and economical guide for tooth preparation for indirect restorations to enable preparation with better planning of margins and occlusion; and cementing the restoration thereafter in single appointment.
Methods: Diagnostic impression and bite registration of the patient was made and casts were mounted on an articulator. Tooth preparation was done on the die obtained, using modified burs with guiding cone attached to shank, and restoration was fabricated over it. The path followed by bur during preparation was traced in wax, through the guiding cone. Many such wax tracings were made for different steps of preparation. Impression of tracings was made and poured in resin. Finishing and polishing of set resin obtained was used as guide for tooth preparation for that tooth. The guide was adapted on surrounding teeth and vestibule of the patient and same modified bur was attached to airotor, inserted and moved along it. After using all the guides, in the planned sequence on that tooth, the same tooth preparation was obtained as done earlier on the cast.
Results: The restoration fabricated on the die used for making guide perfectly fitted on the tooth prepared using the guide.
Conclusions: This simple technique will reduce the clinical steps involved in tooth preparation, although laboratory steps will increase. Since it is much more economical than CAD-CAM used for this purpose, it is significantly useful in developing countries with low average income in Asia. Also, given abundant man-power in countries like India and China, laboratory technicians are easily available at lower costs.
This techniaue will also allow preparations to be more conforming to occlusion and placement of margins, especially in malposed teeth, as preparations will be done on casts mounted on articulators. Temporary restorations will soon be a thing of past, and poorer sections of society in Asia will recieve high quality indirect restorations.
