Biomarker for Asymptomatic Apical Periodontitis in Gingival Crevicular Fluid: aMMP-8
Objectives: Gingival crevicular fluid (GCF) is emerging as a new diagnostic aid in assessing apical periodontal tissue metabolism. The aim of this study was to assess the levels of active MMP-8 (aMMP-8) in GCF collected from teeth with asymptomatic apical periodontitis (AAP), their correlation with the lesion’s characteristics and the diagnostic accuracy of the method.
Methods: GCF samples were obtained from teeth with AAP (n=31) and their contralateral teeth as controls (n=31) following the assessment of a clinically healthy marginal periodontium. The diagnosis of AAP and the determination of healthy periodontal tissues in the contralateral teeth were established with clinical and cone beam computed tomography parameters. The lesions’ characteristics in terms of volume, diameter and borders were assessed. Pool GCF samples per tooth were obtained from the whole crevice’s perimeter (mesiovestibular, vestibular, distovestibular, distolingual, lingual and mesiolingual). aMMP-8 levels were determined by ELISA. Non-parametric statistical tests were performed, as the distribution of the parameters was not normal.
Results: Levels of aMMP-8 were higher in the GCF, collected from teeth with AAP (Z=-3.45, p<.001). The healthy controls demonstrated biomarker levels corresponding to a healthy periodontal tissue. Lesions with greater volume showed correspondingly larger diameters. No correlation between aMMP-8 levels and lesions volume, diameter or borders was discovered.
Conclusions: The results of this study suggest that the GCF composition is modified by AAP. GCF can be a potential diagnostic tool for the assessment of teeth with AAP and clinically healthy marginal periodontium. aMMP-8 can be used as a biomarker in GCF for the assessment of AAP.
