Perception and Oral Health Related Quality of Life of Maxillary Midline Diastema among Nigerians of Different Ethno-social Strata

Objectives: To determine the perception of Maxillary Midline Diastema (MMD) among Nigerians of different ethno–social strata and assess the OHRQoL of individuals with MMD.
Methods: A national survey recruited 1500 participants from the six geo-political zones in Nigeria using multi-stage sampling technique.
A pictorial grading of MMD was developed using Photoshop version CS6 software to create posed smiles with ‘no MMD’, ‘mild MMD’ (1-2mm), ‘moderate MMD’ (3-4mm) and ‘severe MMD’ (≥5mm). The digitally manipulated smiles were randomly incorporated into a questionnaire used to rank respondents’ perception of MMD.
Participants with MMD were identified, their occlusal status assessed using Dental Aesthetics Index (DAI). Impact of MMD on their OHRQoL was determined using Psychosocial Impact of Dental Aesthetics Questionnaire (PIDAQ).
Descriptive and inferential statistics were carried out using Stata version 12. Statistical significance level was set at p〈0.05.
Results: Smile with ‘mild MMD’ (22.3±4.2) was the most attractive, while smile with ‘severe MMD’ was least attractive (7.7±5.4). Age group, religion, educational status and ethnicity contributed significantly to ranking of MMD categories (p<0.0001). Young adults and Bini ethnic group recorded the highest ranking for ‘mild MMD’(p<0.0001). Raters with tertiary education and of Igbo ethnic group reported the highest mean ranking for smile with ‘severe MMD’ (p<0.0001).
A total of 463 participants had MMD with mean DAI score of 30.7±7.74 indicating an elective need for orthodontic treatment. Their mean PIDAQ score was 29.02±14.31 suggestive of positive impact of MMD on OHRQoL. Female gender (p<0.05), Christianity and Igbo ethnic group (p<0.0001) showed positive impact on OHRQoL.
Conclusions: Mild MMD was the most attractive to the majority of Nigerians while the least attractive was ‘severe MMD’. Age group, religion, ethnicity and level of education contributed significantly to MMD preference. MMD was positively associated with improved QoL among the Igbo ethnic group, Christianity and female gender.
