Long-term Outcome of the IMZ Implant System

Objectives: To evaluate radiographic and peri-implant outcome of intramobile cylinder implants (IMZ) after at least 23 years.
Methods: From 94 documented cases of patients treated with IMZ implants between 1981 and 1995, 29 patients had died and 26 were unavailable. 39 patients were successfully contacted of which 16 patients with at least one implant still in situ agreed to participate in a clinical examination. One patient refused the radiographic examination, remaining the inclusion of 15 patients with 32 implants for the present study. Information on implant status, oral and general health was collected. Pocket probing depth (PPD), bleeding on probing (BOP) and plaque index (PI) were evaluated and baseline data on marginal bone level were compared to data collected at the present follow up. Possible correlations between radiographic and clinical parameters were identified using Spearman-Rho test.
Results: Eight patients were treated with bar-retained mandibular overdentures and seven patients had fixed partial dentures. After a mean observation time of 29 ± 3 years no implant was lost. The mean marginal bone loss was 2.5 ± 1.9 mm. The overall average PPD was 2.41 ± 1.48 mm, while mean values for BOP and plaque index were low. Marginal bone loss correlated with PPD at respective sites (P<0,05) as well as with BOP (P<0,05). There also was a statistically highly significant correlation between PPD and BOP (P<0,001)
Conclusions: Implant treatment with IMZ-System appears to show highly favorable long-term results over decades. Our findings suggest, that, if peri-implant healthy conditions can be maintained marginal bone changes after initial remodelling were minimal. But due to the high drop-out rate results cannot be generalized. As data on implants and peri-implant conditions with comparable follow up periods are scarce, further studies on this subject are needed.
