Peri-implant Gingival Recessions Around Immediately Loaded Implants with Platform Switching

Objectives: Peri-implant gingival recessions (Rec) are associated with tissue phenotype, implant position and bone levels. However, there is not information about the Rec around immediately loaded implants with different implant diameters and platform switching (PS).
Methods: 18 patients with edentulous mandible were included. After bone reduction, two groups of implants were placed intra-foraminalrandomly (left or right) and after connection with final prefabricated abutments implants were splinted together with a provisional cementedbridge. In one group, 3 implants with 3.5mm (group A) and in the other 3 4.8mm diameter implants (group B) were placed per patient using a prosthetic guide and all of them with PS. Impressions from abutment level using silicone materialwere taken after removal of the restorationsin 3(final prosthesis delivery), 6, 12 and 24 months of loading.Measurements (in mm) on study castsfrom abutment shoulderto soft tissue height in 4 areas (mesial, distal, mid-buccal and mid-lingual) were performed using a calibrated periodontal probe.Zero or positivenumbers represented no recession and negative values demonstrated Rec.Statistical analysis was performed usingWilcoxon-test between different loading periods and Mann-Whitney to compare between two implant systems(at 5%).
Results: Due to lack of osseointegration (3 implants) and relocations (2 drop-outs) we calculated following mean values (±SD) in mesial, middle, distal implants for 3, 6, 12 and 24 months, respectively. Group A mesial-implants: 0.17±0.66, -0.08±0.76, -0.10±0.65, 0.04±0.38; middle-implants: -0.12±0.92, -0.40±0.76, -0.42±0.75, -0.27±0.68; distal-implants: -0.21±0.96, -0.29±0.89, -0.27±0.67, -0.17±0.59. Group B mesial-implants: -0.63±1.01, -0.69±0.86, -0.48±0.79, 0.23±0.58; middle-implants: 0.54±1.02, -0.60±0.97, -0.60±0.68, -0.40±0.58; distal-implants: -0.21±0.94, -0.10±1.07, -0.04±0.80, 0.0±0.6 (p-values >0.05).
Conclusions: Soft tissue behavior around implant systems with PS is similar under immediate loading conditions without abutment removal. The soft tissue changes are similar around immediately loaded implants independent on the implant diameter and are not influenced by crestal bone levels.
