Evaluation of Periodontitis Before and After Labor in Overweight/Obese Women

Objectives: This study aimed to evaluate the systemic and periodontal conditions, and its impact on quality of life, of overweight/obese women during pregnancy and after labor.
Methods: In the second trimester (T1), 93 pregnant women were divided into two groups with either excessive weight (G1, n=53) or normal weight (G2, n=40) and subsequently examined them in the third trimester of pregnancy (T2) and at least 2 months after labor (T3). The following variables were analyzed: a) systemic impairments during pregnancy - arterial hypertension [AH] and gestational diabetes mellitus [GDM]; b) weight gain during pregnancy; c) oral hygiene behavior; d) periodontal conditions; e) quality of life related to oral health in T1 and T3 [Oral Health Impact Profile - OHIP-14]; f) systemic health condition after pregnancy. T test, Mann-Whitney, chi-square, ANOVA and the binary logistic regressions were adopted (p<0.05).
Results: G1 showed higher frequency of GDM and AH in T1 and T2, respectively (p=0.047; p=0.004). G1 showed weight gain higher than expected (p=0.019). Both groups had worse oral hygiene behaviors after labor. A higher frequency of periodontitis was found in all periods for G1 (p<0.05). G2 showed improvement of all periodontal parameters after labor, whereas G1 showed no difference regarding these parameters between time periods. There were no differences between groups in T1 and T3 regarding OHIP-14 overall score. However, the OHIP-14 overall score improved after delivery for both groups (p<0.05). During T3, G1 had a higher risk of developing cardiovascular disease (p=0.024). Logistic regression pointed out that excessive weight and low socioeconomic level are directly associated with periodontitis.
Conclusions: In conclusion, pregnant women with excessive weight present worse systemic and periodontal conditions during pregnancy and after delivery, and periodontitis is closely related to excessive weight. In addition, worst oral condition during pregnancy negatively impacts quality of life of the patients.
