A Retrospective Study of Effects of Corticosteroids on Dental Implant Failure

Objectives: The objective of this project is to determine a relationship between corticosteroid use and rate of dental implant failures placed in the post-doctoral clinic at the University at Buffalo School of Dental Medicine. It is hypothesized that corticosteroid use leads to higher rates of implant failures, as the immunosuppressive nature of corticosteroids may interfere with the osseointegration and the healing process of implant placement.
Methods: A retrospective chart review of patients who received dental implant placement at University at Buffalo School of Dental Medicine between January 2008 and December 2016 was performed. Information regarding patient use of corticosteroid medications, date of implant placement, date of implant failures (leading to loss of implants), and other confounding parameters (such as smoking and diabetes status) were collected and recorded. The uses of corticosteroid medications were self-reported by patients in their charts. The main outcome for the study was dental implant failures. Duplicate measurements were performed for a randomly selected group of ten patients to assess intra-examiner reliability with at least one-week interval. Statistical analysis was assessed at statistical significance at the 0.05 level.
Results: This study has currently reviewed 208 patient charts, for which there have been 362 implant placement cases. Out of the 208 total patients, 15 patients have reported failures for 15 implant sites. The study shows that that 20 of the 208 patients were taking chronic corticosteroids at the time of implant placement from the reviewed charts, of which 3 patients experienced implant failures.
Conclusions: At this time, the implant failures associated with chronic glucocorticoid use has accounted for 20% of the total implant failures in the study. More charts will continued to be analyzed to increase the sample size.
