Color Stability of CAD/CAM Materials After Accelerated Staining and Bleaching

Objectives: To assess the color changes of new composite-resins and hybrid-ceramics for CAD-CAM technology induced by coffee immersion and bleaching with 10% carbamide-peroxide gel.
Methods: Sixty 1mm-thick samples (A3 shade, high and low translucency) were obtained from the following CAD-CAM materials (n=5): composite-resins- Lava Ultimate, Crios Brillant, Shofu blocks; hybrid-ceramics- VITA Enamic; feldspathic ceramics-VITA Mark II, and lithium-disilicate ceramics- e.max CAD; in addition, five samples of direct composite-resin were fabricated (Filtek Ultimate A3). Samples were stored for one week in a coffee solution. Next, the samples underwent an at-home bleaching procedure (10% carbamide-peroxide gel, 6h/day, five days). CIELAB parameters were recorded on black background at baseline, after coffee immersion and after bleaching and whiteness index (WID) was calculated. Influence of staining and bleaching on CIELAB and WID was assessed with repeated-measures ANOVA (α=0.05). Color differences after staining and bleaching were calculated using ΔE00 formula and compared between materials (one-way ANOVA and Tukey's post-hoc tests, α=0.05).
Results: Except e.max CAD, CIELAB color parameters and WID varied significantly after staining and bleaching for all tested materials (p<0.05). The greatest color changes after staining were calculated for Lava Ultimate HT (ΔE00co_i=3.05), Filtek Ultimate (ΔE00co_i=2.46) and Crios Brillant HT (ΔE00co_i=2.20). VITA Enamic, VITA Mark II and e.max CAD showed the smallest changes (below the perceptibility threshold of 0.8). Although the color change of Shofu block samples (ΔE00co_i=1.65 for HT and ΔE00co_i=1.20 for LT) was not as significant as in the case of Lava Ultimate or Crios Brillant, bleaching procedures had the smallest effect upon these samples. HT samples showed higher color differences than LT for all materials tested.
Conclusions: All CAD-CAM composite-resin materials underwent significant modification in color after coffee staining that was above the clinically perceptible threshold. However, bleaching procedures demonstrated to be effective in stain removal for these materials. Hybrid ceramics, lithium-disilicate and feldspathic ceramics were the most color stable materials.
