Daily vs Weekly Denture Cleanser Regimens: Denture Plaque and Microbiology
Objectives: Dentures harbour complex biofilms, so cleaning is considered essential to good oral hygiene. However, there is a lack of evidence on the efficacy of treatments frequencies. This study compared the changes in microbial counts, microbial composition and denture plaque on full dentures following the daily or weekly use of a denture cleanser. Methods: 2 period, examiner-blind, randomised, 7-day crossover study of complete maxillary denture wearers. Microbial counts and composition were investigated by sampling four quadrants lateral to the midline and corresponding to the palatal rugae. A pre-sterilized filter paper disc was lightly pressed against the quadrant for 20 seconds prior to aseptic removal. Discs were processed for microbial culture [agar plate count, colony forming units (CFU)/disc] and for molecular microbial analysis (qPCR with specific primers for 8 oral microbial groups). Denture plaque was assessed using the Clinical Categorization of Denture Cleanliness Index. Subjects used Corega® Tabs as pack usage instructions either daily (7 x single treatments) or weekly (1 treatment at day 7). On remaining days, weekly treatment subjects cleaned their dentures with water alone Results: At Day 7, daily and weekly treatments resulted in values of 0.26 Log10(CFU/disc) and 1.06 Log10(CFU/disc) aerobic bacteria respectively, and 0.50 Log10(CFU/disc) and 0.93 Log10(CFU/disc) for anaerobic bacteria. A statistically significant between treatment difference was observed for aerobic bacteria microbial count, with the greater reduction observed for the Daily use treatment (-0.86 mean difference, p=0.0144). For anaerobic bacteria, a directional between treatment difference was observed in favour of the daily use treatment, however this difference was not statistically significant (-0.48 mean difference, p=0.1879). At Day 7 there was a statistically significant difference in denture plaque reduction in favour of daily vs weekly treatment. Conclusions: Daily treatment with denture cleanser tablets reduced denture microbial numbers and plaque compared with weekly treatment.
