Is PET/CT Useful for Early Detection of MRONJ?

Objectives: The purpose of this study is to investigate whether F-18 fluoride and F-18 fluorodeoxyglucose positron-emission tomography and computed tomography (PET/CT) scan images are useful for early detection of medication-related osteonecrosis of the jaw (MRONJ) and determination of the curative effect.
Methods: We investigated retrospectively using PET/CT images taken by follow-up to three breast cancer patients who were diagnosed with multiple bone metastasis of breast cancer and developed MRONJ after administration of zoledronate and denosumab.The correlation between the state of MRONJ and the PET/CT image was examined. The SUV-MAX value of the jawbone before MRONJ symptomatic bone exposure and pain occurred and the transition of the SUV-MAX value after treatment for antibiotic administration started were observed and evaluated over time.
Results: There was accumulation of PET/CT in the jawbone area before pain and bone exposure which is a symptom of MRONJ occurred in all three cases. In addition, SUV-MAX declined when conservative treatment of MRONJ was started.
Conclusions: PET/CT may be useful for early detection of MRONJ. PET/CT may be a diagnostic tool to judge the effect of treatment for MRONJ.
