Monitoring Regression of Post-Orthodontic Lesions With Impedance Spectroscopy
Objectives: The aim was to evaluate if Alternating Current Impedance Spectroscopy (ACIS) can be used to quantify the regression of post-orthodontic white spot lesions over a 16-week period. Methods: The study group consisted of 28 adolescents with various stages of white spot lesions after treatment with fixed orthodontic appliances. Up to 8 affected anterior maxillary surfaces were included per patient (189 in total) and scored immediately after debonding (baseline) with the ICDAS system and the ACIS technology (CarieScan Pro). After 8 and 16 weeks the patients were reexamined by one of 3 clinicians. Prior to the study, inter- and intra-class correlation (ICC) was calculated from findings of 30 extracted teeth that were independently scored and rescored by each examiner. The ASIC technology was analyzed in a mixed statistical model considering systematic effects of visit, tooth and observer, random effects of patients and visits within patients. An additional autoregressive structure was used to correlate repeated measurements on the same tooth. ICDAS findings were processed with ordinal regression. Results: Intra- and inter-examiner agreement ranged between 0.65-0.86 and 0.53-0.82, respectively. The estimate for the ASIC technology was 55.5 at baseline and decreased significantly (p<0.05) to 46.9 after 8 weeks. No significant difference was found between 8 and 16 weeks. When ICDAS findings were grouped into three categories (0, 1 and 2+3) the odds for being in a high category was 18-times higher (p<0.001) at baseline when compared to the 16-week follow-up. The corresponding odds between the first and second follow-up was 6 times higher (p<0.001) Conclusions: The ACIS technology could be used to monitor the initial and most profound regression of post-orthodontic white spot lesions but was unable to unveil the smaller changes detected by the visual ICDAS system between 8- and -16 weeks.
