Functional Dentition in Brazilian Elderly People and Associated Factors: A Multilevel Generalized Structural Equation Modeling Approach

Objectives: The objective of the present study was to evaluate the association of Socio-Economic Status (SES) as a latent variable in mediation effects on Oral Health Related Quality of Life (OHRQoL) through four definitions of Functional Dentition (FD) of Brazilian elderly participants in the São Paulo Oral Health 2015 (SBSP-15) survey.
Methods: In this study, four sets of multilevel generalized structural equation models (GSEM) were used; 1- WHO Functional Dentition (FDWHO); 2- Well-distributed teeth (WDT); 3- FDclass5 and 4; FDclass6, to mediate indirect effects of SES on Oral Health-Related Quality of Life (OHRQoL) in 5951 elders. The latent variable (OHRQoL) was extracted from the Oral Impacts on Daily Performances (OIDP) questionnaire, and GSEM was used to estimate the direction of association among the variables.
Results: SES was directly and strongly associated with FDWHO (Total effects [SC]=0.56, p<0.001); WDT (Total Effects [SC]=0.55, p<0.001); FDclass5 (Total effects [SC]=0.07, p<0.001) and FDclass6 (Total effects [SC]=0.05, p=0.001). All FD outcomes mediate effects of SES on OHRQoL (p<0.05). SES had a direct effect on Hapiness ([SC] -0.28, p<0.001); Dental Services use ([SC] -0.28, p<0.001) and Reason for services use ([SC] -1.36, p<0.001). Hapiness had a direct effect on ORHQoL ([SC]=0.05, p<0.001) .
Conclusions: SES was associated to all FD outcomes. FD was a good e endogenous variable to mediate effects by SES on OHRQoL.
