Comparison of Intracanal and Gingival Crevicular Fluid Biomarkers in Endodontic Disease: A Pilot Study
Objectives: The purpose of this study was to measure biomarkers in the intracanal fluid (ICF) and GCF of teeth undergoing root canal treatment (RCT).
Methods: Twelve teeth needing RCT and 12 healthy contralateral teeth were included in the study. Clinical data and endodontic diagnoses were recorded. Pain scale levels were measured using VAS. GCF was collected from teeth needing RCT and from healthy contralateral teeth. ICF was also collected after accessing the teeth for RCT. Forty-one immune-related cytokines were measured using multiplex bead-based immunoassay technology. Statistical significance was tested using multiple regression analysis.
Results: Thirty-six cytokines were measurable in varying amounts in the ICF and in the GCF of both normal and diseased pulps. G-CSF, VEGF, TGF-α, PDGFAB/BB, IFNα2, IL-1RA, EGF, IL-1 α, RANTES and IL-6 were the ten most expressed cytokines. TGF-α, IFNα2, PDGF-AB/BB and RANTES yielded a similar expression in the intracanal fluid and in the GCF of the same endodontically diseased tooth while their expression in the GCF of teeth with normal pulps was significantly lower.
Conclusions: These results provide a promising future for GCF biomarkers to be used as an objective, non-invasive and biologically-based diagnostic tool in determining the pulpal status. Further studies analyzing thousands of GCF biomarkers from larger sample sizes with pre-operative, post operative and recall GCF samples are strongly encouraged.
