Is Periodontitis Related to Incidence of Cancer Among Dentate Adults?
Objectives: We studied if periodontitis measured as number of teeth with gingival pockets≥ 4 mm was related to the incidence of cancer.
Methods: Baseline data came from the nationally representative Health 2000 Survey including dentate Finnish adults aged 30+ years whose periodontal health was measured in 2000/2001. Number of teeth, number of periodontal pockets and presence of plaque were measured during clinical examination. Information on smoking and use of alcohol came from interviews. The cancer statistics were obtained from the Finnish Cancer Register and classified according to the international classification ICDO3-code of the primary cancer site. Occurrence of all cancer types was followed until December 31, 2012. The data were statistically analyzed using Cox Proportional Hazards Modeling.
Results: Number of teeth with deepened periodontal pockets was not associated with occurrence of any cancer during the follow-up when adjusted for age, sex, smoking, alcohol consumption, presence of plaque and number of teeth at baseline. In this model, the only significant predictor was age (HR 1.1, 95%CI 1.0-1.1). Lower number of teeth associated with occurrence of cancer in respiratory system (HR 0.95, 95%CI 0.91- 0.99) and higher number of teeth with skin cancer (HR 1.0, 95%CI 1.0-1.1) and cancer in eyes and central nervous system (HR 1.2, 95%CI 1.0-1.3) when adjusted age, sex, smoking, alcohol consumption, presence of plaque and number of teeth at baseline. Number of teeth nor number of teeth with deepened periodontal pockets did not associate with any other cancer.
Conclusions: This study does not provide evidence that periodontitis can be considered as an independent risk for cancer in a general adult population.
