Effect of Maternal Education on Adolescent’s Gingivitis: A Birth Cohort

Objectives: To test whether maternal education have a direct effect on gingival bleeding of adolescents aged 12, In addition, we aimed to assess whether oral health behaviours mediate the relationship between early life socioeconomic conditions and adolescents’ gingival bleeding.
Methods: Data from two oral health studies nested in the 1993 Pelotas (Brazil) population-based birth cohort study at participants’ ages 6 (n = 359) and 12 years (n = 339) were used. The proportion of gingival bleeding and the prevalence of gingivitis (cut-off set in the median value) at age 12 were registered. Maternal education at birth was taken as exposure. Toothbrushing frequency, and dental visit at ages 6 and 12 years were postulated as mediators of the association between maternal education at birth and gingival bleeding. Time-varying family income through the life-course was included as later confounder. Paternal education was taken as baseline confounder. The controlled direct effect (CDE) of maternal education at child’s birth on gingival bleeding at age 12 was estimated using Marginal Structural Models (MSM). Additionally, Path Analysis was also employed to estimate standardized direct, indirect and total effects of maternal education on adolescent’s gingival bleeding.
Results: Adjusted analyses using MSM showed adolescents whose mothers had less than 8 years of schooling presented 3.70 higher risk of gingivitis (Rate Ratio _ RR 3.70; 95%CI 1.68-8.19). Low maternal schooling doubled the proportion of gingival bleeding at age 12 not mediated by dental visit and toothbrushing frequency (RR 2.02; 95%CI 1.54-2.65). Path Analysis also revealed that maternal schooling had a direct effect on gingival bleeding.
Conclusions: Low maternal education has a direct effect on adolescents’ gingival bleeding not mediated by oral health related behaviors. Results were similar independently of the analytical approach used.
