Periodontal Disease and Quality of Life: Impact of Dental Fear
Objectives: The impact of oral health-related quality of life (OHRQoL) has been studied extensively and remains an important factor in dental research. Researchers, however, have conducted only a few studies to examine the mediating effect of psychosocial phenomena between disease and OHRQoL. More specifically, there is a dearth of research examining the impact of psychosocial factors on the relationship between periodontal diseases and OHRQoL. Methods: The present study focused on data from Appalachian adult participants (n = 895) from the Center for Oral Health Research in Appalachia (COHRA1) family-based cohort. Data were derived from a modified Periodontal Screening and Recording Index (PSR) focusing on sextants, responses to the Dental Fear Survey, Fear of Pain Questionnaire-9, and the Oral Health Impact Profile-14. Confirmatory factor analyses and structural equation modeling were used to estimate the effects of clinical periodontal disease screening indicators on oral health-related quality of life. Additionally, the role of dental fear in mediating the relationship between the PSR results and OHRQoL was assessed, while accounting for the variance due to fear of pain and demographic variables. Results: A significant total effect was found between PSR sextant five (mandibular anterior), avoidance and anticipatory fear, and indicators of OHRQoL (Psychological β = .185, p < .001; Pain β = .128, p = .010; Functionality β = .145, p = .007). Anticipatory fear and physiological arousal subscale scores of the Dental Fear Survey help to predict factors of the OHIP-14. Other significant associations between sextants, dental fear, and OHIP scores were found. Conclusions: Findings confirm the relation of periodontal disease indicators and OHRQoL. Dental avoidance and anticipatory fear mediates oral health-related quality of life in patients with indicators of periodontal disease in a sextant that may be visible and noticeable in terms of periodontal involvement.
