12-months Prospective Study on Two-piece Transmucosal-implants in the Aesthetic Zone

Objectives: Aim of this prospective cohort study was to evaluate clinical outcome, Marginal-Bone-Loss (MBL), Pink-Esthetic-Score (PES), Crown-Height (CH) and periodontal parameters of a new transmucosal two-pieces implant inserted in the esthetic zone characterized by a new neck with hyperbolic-profile.
Methods: A total of 37 ZirTi implants (Prama, Sweden and Martina, Due Carrare, ITALY) were inserted in 32 consecutive healthy non-smoker patients (18F;14M; 57±11yrs) to replace single-teeth in the aesthetic area (15-25).
Eleven implants were placed immediately after extraction (immediate-group), 14 were placed after 2-3 months from extraction (early-group) and 12 implants were placed after 12 months from extraction (delayed-group) with flapless technique and with chamfer emerging from gingival level for approx. 1,5-2.0mm.
An impression was obtained after 3 months and a custom abutment was applied with the abutment-implant junction approx. 1.5-2.0 mm from gingival margin. A provisional-resin crown was cemented free from tissue compression. A definitive crown was cemented after 2-3 weeks.
Mesial and distal MBL was measured with standardized periapical radiographs after 1-3 months from insertion (pre-loading time) and at 6-12 months after loading. Survival rate, periodontal parameters, crown height (CH) and gingival thickness were measured at the same time.
PES was assessed by one calibrated operator using a 0-1-2 scoring system, 0 the lowest, 2 the highest value. Seven variables were evaluated vs. a natural reference tooth.
Results: After 12 months all implants resulted osseointegrated with no complications. Table shows MBL values (mm±SD) at 1,3,6,12 months after placement.
CH and periodontal parameters resulted stable at all evaluations. PES improved from 6 to 12 months.
Conclusions: The hyperbolic geometry of the transmucosal neck allows to tailor the position of the crown margin and to obtain an aesthetic natural crown emergence profile. Limited MBL values were observed as no compression on mucosal tissue was caused by the crowns.
