GBR With D-PTFE Ti-reinforced Membranes Versus Ti-meshes and Collagen Membranes
Objectives: The aim of this randomized clinical trial is to compare 2 different approaches for Guided Bone Regeneneration (GBR) in the posterior region of atrophic mandibles, in order to find differences regarding the reconstructed bone volume. Methods: Forty partially edentulous patients who required a three-dimensional bone regeneration and implant-prosthetic rehabilitation were randomly divided into two groups and consecutively treated using d-PTFE titanium-reinforced membranes (group A) or titanium meshes covered by cross-linked collagen membranes (group-B). Bone augmentation and implant surgery were performed simultaneously according a GBR technique (t0). A mixture 50:50 of autogenous bone and bone allograft was used. After 9 months of submerged healing, reopening surgery was performed (t1). All surgical and healing complications (SC and HC), initial (t0) and final (t1) bone defect (IBD and FBD), vertical bone gain (VBG) were recorded and analyzed. Results: 99.0% of implants showed an optimal primary stability, with insertion-torque >25Ncm and ISQ >60. All implants achieved a successful osseointegration. In the group A, SC and HC rates were 5.0% and 15.9% respectively; while, in the group B, SC and HC rates were 15.0% and 20.0%. In group A, IBD and FBD were 3.8 ± 1.5 mm (range 2.0 – 6.5 mm) and -0.5 ± 0.9 mm (range -1.8 – 1.5 mm ) respectively; as consequence, in group A the VBG was 4.3 ± 1.2 mm (range 2.3 – 6.5 mm). In group B, IBD and FBD were 3.9 ± 1.6 mm (range 1.8 – 5.8 mm) and -0.3 ± 0.8 mm (range -1.3 – 1.3 mm), respectively; while, VBG was 4.2 ± 1.1 mm (range 1.8 – 6.8 mm). Conclusions: d-PTFE membranes and titanium meshes plus collagen membranes have similar results regarding HC rates, IBD and VBG. GBR with both techniques could be considered a viable solution for restoration of atrophic posterior mandible.
