Radiographic Evaluation and Analysis of Pre-eruptive Intracoronal Resoprtion
Objectives: Coronal resorption occurs in about 1.5-3% of impacted teeth. These lesions have been described as caries, internal resorption and developmental anomaly. Our objectives were to analyze the radiographic presentation of the lesions in both conventional and CBCT images. We also investigated any local and systemic factors that may contribute to the coronal resorption of these impacted teeth. Methods: We analyzed three cases on intra-coronal resorption and also conducted a literature review to examine any contributing local and systemic factors. Articles were explored to recognize any association with pain, local infection (ex. pericornitis) periodontal disease, inherited conditions, xerostomia, smoking or any reported other systemic disease. Results: The radiographic features include well-defined radiolucent lesions in the dentin adjacent to the dentinoenamel junction near the occlusal surfaces of the crowns of impacted teeth. A comparison of medical histories of our three patients revealed associations with hypertension, head or neck trauma, and aspirin consumption. Both of the female patients have histories of breast cancer or breast calcifications. All three patients have an extensive number of restorations; almost posterior teeth have occlusal fillings and two of the patients were missing multiple posterior teeth. The literature review suggests local factors such as pressure from ectopic position or an abutting adjacent tooth may contribute to coronal resorption. Conclusions: Our findings suggest that both local and systemic factors contribute to coronal resorption.
