A. actinomycetemcomitans Serotypes Associate With Periodontal and Cardiac Status

Objectives: Aggregatibacter actinomycetemcomitans is linked to aggressive periodontitis and may play a role in systemic diseases. We investigated the association between the A. actinomycetemcomitans serotypes, periodontal status, and coronary artery disease (CAD).
Methods: The study population included 495 patients with angiographically verified CAD status: 36.4 % with stable CAD, 33.7% with acute coronary syndrome (ACS), and 24.2% with no CAD. All patients had a detailed clinical and radiographic oral health examination. The serotypes were determined by using qPCR.
Results: The number of A. actinomycetemcomitans serotype-positive subjects was 42 (8.5%). Serotype frequencies were as follows: 35.7% serotype c, 28.6% b, 26.2% a, 7.1% e, 2.4% d, and 0% f. Two (4.8%) patients harboured two serotypes.

Serotypes b (p=0.021) and c (p<0.001) associated significantly with periodontal inflammatory burden index composed of deepened periodontal pockets. Serotype a did not associate with deepened pockets, and none of the serotypes associated with bleeding on probing.

In general, saliva and subgingival bacterium quantity associated with serotypeable A. actinomycetemcomitans, and the quantities were highest in patients harbouring serotype c. Generally, serum IgA and IgG antibody levels against the bacterium associated with serotypeable A. actinomycetemcomitans; IgG levels did not specifically associate with any serotype, but low IgA levels associated with serotype a.

Majority of the serotypeable patients had stable CAD (52.4%) compared to ACS (28.6%) and no CAD (19.0%). Serotypes b and c were more frequent (59.3%) in patients having CAD (p=0.076) than other serotypes. Also, the severity of CAD (p=0.038) associated with serotypes b and c, which were more frequent with patients having stenosis in one artery (48.1%).

Conclusions: A. actinomycetemcomitans serotypes b and c associated with both periodontal and cardiac status. All serotypes associated with the quantity and the serology of the bacterium emphasizing both local and systemic effect of the A. actinomycetemcomitans serotypes.
