Comparison of FACIT, ESSDAI, OSDI in Sjogrens and Sicca Patients
Objectives: Compare of the level of fatigue, ocular dryness and disease activity in Sjogren’s Syndrome patients diagnosed by positive lip biopsy, Sjogren's patients diagnosed by positive Sjogren’s specific antibody (SSA), and Sicca patients. Methods: With IRB permission, the records of Tufts Oral Medicine 138 patients presenting with Sicca (n=91) and diagnosed Sjogren’s Syndrome (n= 47) were reviewed . Among the Sjogren’s Syndrome patients, 27 were SSA positive and 20 were lip biopsy positive with positive Schirmer or positive sialometry or both. The EULAR Sjogren’s Syndrome Disease Activity Index (ESSDAI) was administered to assess disease activity, Ocular Surface Disease Index (OSDI) used to assess ocular dryness and Functional Assessment of Chronic Illness Therapy (FACIT) used to assess fatigue were completed by the patients. All questionnaires had been validated in Sjogren's patients. Results: The gender breakdown was 91.2% female for the Sicca patient group, 88.9% female for the SSA+ group, and 95.0% female for the lip biopsy+ group. Table 1 shows the age, ESSDAI, FACIT, and OSDI scores by group. The Kruskal-Wallis test of OSDI across the groups was significantly different p=0.030 with the SSA+ group being higher than the Sicca group. p=0.025The Kruskal-Wallis tests of the ESSDAI central nervous system problems , constitutional issues and glandular across the groups were significantly different p = 0.004 , p = 0.027 and p=0.037 respectively with the lip biopsy group being higher than the Sicca group in these tests. There were no statistically significant differences in FACIT or total ESSDAI score among the groups. Conclusions: Statistically significant differences were seen on several of the disease activity measurement scales of ESSDAI and OSDI among some of the groups.These findings suggest that lip biopsy as well as SSA+ Sjogren's patients might be appropriate to include in clinical trials. Further analysis with a larger study population is necessary.
