Association of Periodontitis With β-cell Function and Insulin Resistance: NHANES

Objectives: The purpose of this study is to investigate the association of chronic severe periodontitis (exposure) with pancreatic β-islet cell function and IR (outcomes) in U.S. adults ages 30+ years who are diabetes-free.
Methods: We used National Health and Nutritional Examination Survey data from 2009-2012 to analyze the Homeostasis Model Assessment (HOMA) of IR and β-cell function in dentate US adults. After excluding participants with missing data for variables of interest, 1,523 and 1654 participants were included for analysis of HOMA-IR and HOMA-β, respectively. Periodontal status was defined using the Centers for Disease Control and Prevention/ American Academy of Periodontology criteria, which classifies periodontitis as none, mild, moderate, and severe based on attachment loss and probing pocket depth. We identified and operationally defined all covariates and outcome variables. SAS survey analysis procedures were used for bi-variable and multivariable analysis. Multivariable logistic regression analyses evaluated the association between periodontitis and HOMA-IR and HOMA-β, while accounting for other covariates.
Results: Severe periodontitis was not significantly associated with insulin resistance (OR: 0.863; 95% CI, 0.515-1.446; p=0.56), nor with beta cell function (OR, 0.993; 95% CI, 0.594-1.660; p=0.978). However, the covariates age, sex, BMI, presence of prediabetes, and metabolic syndrome were significantly associated with HOMA-IR. The covariates age, income, race-ethnicity, BMI, and metabolic syndrome were significantly associated with HOMA-β.
Conclusions: Our study found that severe periodontitis was not associated with insulin resistance and β-cell function in these data. Future studies that define periodontitis by including measures for gingival bleeding as an active indicator of inflammation may help to elucidate a possible relationship between periodontal disease and HOMA-IR and HOMA-β.
